Worthy Sir Knights,

Use the below form to submit your list of Candidates. If you have more than 5 Candidates please reach out to the District Secretary at districtsecretary@kofcnc.org to request a spreadsheet to submit the requested information for the next exemplification and any questions you may have about candidates. We ask that you send all Form4's and Dress Code forms via email to the District Secretary prior to sending the forms in the mail.

ONLY THE FAITHFUL COMTROLLER OR FAITHFUL NAVIGATIOR SHOULD BE SUBMITTING THIS FORM. NO CANDIDATES SHOULD BE SUBMITING THIS FORM.

Assembly	This is the Assembly submitting the form. The FC or FN of the Assembly should be filling this out. Only fill out if you are one of the authorized members or have been delegated to do so on their behalf.
