Why Forms Matter: Building Our Council’s Success Through Timely Submissions
In the Knights of Columbus, we are called to Charity, Unity, and Fraternity—but none of those can be lived out effectively without a solid foundation of communication and accountability. That’s why turning in required forms on time is so important. Whether it’s tracking our progress, celebrating our achievements, or ensuring our Councils remain in good standing, these forms serve as both a record and a roadmap for our efforts at the local, state, and national levels.
What Forms Are Required and When?
There are several forms that every Council should be familiar with, but here are a few of the most essential and commonly due:
Form 185 – Report of Officers Chosen for the Term: Due by July 1st. This form ensures Supreme and State officers know who to contact and who is leading your Council. In the State of NC, We ask that you turn it as soon as you complete your elections.
Form 365 – Service Program Personnel Report: Due by July 1st. This form identifies your program directors and leaders—key players in every Council’s success. In the State of NC, we ask that you turn it once you assign your Directors and chairman.
Form 10784 – Fraternal Programs Report: Due monthly/ASAP after an event . This form documents your Council’s activities and charitable work, giving visibility to the great things your team is doing and helping others learn from your example. We are also requesting that you send us pictures of the event to add to our Facebook page as a post about the event.
SP-7 – Columbian Award Application: Due to Supreme by June 30th. This is the final step in securing one of the most honored recognitions a Council can receive. In the State of NC we ask that you turn it in by the State Convention so that we can review it and give you feedback.
Missing or delaying the submission of these forms can impact your Council’s ability to qualify for awards, receive support from the State or Supreme Councils, and even maintain good standing.
Where Can You Find the Forms?
All official forms can be found on the Supreme Council’s website, www.kofc.org, under the “For Members” section. Additionally, you can access them via the NC State Council website at www.kofcnc.org. Look under the “Forms” tab to download blank copies or review submission guidelines.
The Best Way to Submit Forms
Submitting your forms doesn’t need to be complicated. Here’s the most effective method to make sure your submission is received, tracked, and acknowledged:
Complete the form electronically, preferably through Member Management.
Email your completed form to all the following (Always follow up with an email copy to your leadership and the forms team to ensure nothing slips through the cracks).

This three-part submission ensures that your form is acknowledged at every level—from your local leadership all the way to the state tracking team.

Turning in forms may not seem glamorous, but it’s one of the most important administrative tasks we perform. It ensures that your Council is recognized, your members are supported, and your work is celebrated. More importantly, it honors the time, talent, and treasure that your brother Knights contribute in service to our Church and communities.
